Instructions:

 

1. In order to participate in STEMConnect, all participants must complete the Consent and Release Forms, including the Medical Release and Pickup Authorization.

 

 

2. Click on and download the PDFforms below (look also for an email reminder).

 

 

3. Print, read, and complete the forms with all the requested information including a dated signature from the participant's parent or guardian. 

 

 

4. Have the participant bring the forms to Day 1 of the camp. Or you can scan and email the completed form to STEMConnect5242@gmail.com.
